Think about this statement and you will see that you need to get back into training one step at a time.
Firstly myself I would make sure that I could make time for the training at a regular time/interval. I normally train before I go to work (meaning that I train at 06:00 on my early shift and 07:00 on my later shifts), I know this isn't the ideal time for most people to train due to being so soon after getting up, but it allows me to train regularly and at the same time of day. Once you get used to it, it is not a problem.
Secondly I would wait until I had the regular training sorted before trying to concentrate on diet. Without constant regular training you can't sort out the correct diet. After taking a while to get over your injuries and into regular training your motivation should have improved enough allowing you to concentrate on diet. If you're eating well, then you should still grow once your training takes off, so the diet becomes a way of fine tuning everything.
It may be a good idea to look at some positive thinking exercises or philosophy. A friend studying "sports psychology" once told me that the most important part of an athlete is the top two inches, meaning it is what goes on inside the brain of an athlete that decides how good the performance is!
To be successful you need your ABC's; Application-belief-commitment. If any one of these are missing you will struggle to achieve your goals. Setting short term goals will also help you to get back on the right road one step at a time.
