There is no law specifying that the nutrition facts panel list raw or cooked servings, it can be either.
However, what you can be sure of, is that the panel will list the servings of the food "as is" unless specified. That means, if you got a package of raw chicken breasts, the nutrition facts will show a raw serving, unless it specifically says cooked on the panel. Conversely if you get a pre-cooked chicken, or are looking at restaurant nutrition facts, the nutrition facts will measure the serving as it has been cooked and prepared, they will not list the nutrition facts for the raw ingredients because you never come in contact with them.
Unfortunately if you are relying on random internet web sites to supply you with nutrition facts, and they do not specify cooked or uncooked, you really have no idea what they are measuring. Even if they do specify, they are many times wrong, as you don't know what their source was and they could have just failed to realize whether their source was showing a cooked or raw serving for that food.
